Health Screen Information
Health Screen paper work available here. The following pages are to be filled out by all prospective student/athletes that are coming out for any sport. There are a total of 11 pages that must be completeley filled out before receiving a health screen. Any paperwork that is not completed properly will be returned to the student/athlete and a medical release will not be issued. If you have any quetions, please ask Vance or Cherrisse.
