All forms required a handwritten signature or digital signature.
In order to process students’ requests, students need to submit the appropriate forms and documents. Missing documents and signature will delay the completion of your request.
All forms and paperwork are processed in the order in which they are received.
Allow up to ten (10) days to process your request – so turn in your requests early!
