Transcripts

OFFICIAL TRANSCRIPTS

To order official transcripts, you may download and print the transcript request form or provide the following information in a letter or fax:

• Name
• Name at the time of attendance (if different)
• Social security number
• Date of Birth
• Dates of Attendance
• Name and Address where transcripts are to be sent
• Number of copies needed
• Your signature

TRANSCRIPT DELIVERY

Based on your ordering instructions, transcripts will be mailed to the address(es) you provide or held for you to pick-up in the Office of Admissions & Records. Please bring picture I.D. with you if you are picking up a transcript in person. TRANSCRIPTS CANNOT BE FAXED.

TRANSCRIPT FEES

Rush processing (available within 3-5 working days*): $5.00 per transcript. Same day service is not available.

Regular processing (available in 10-20 working days*): $3.00 per transcript. If multiple transcripts are ordered on the same request, the first transcript is $3.00, each additional transcript is $1.00. Note: First 2 regular processing official transcripts are free.

Include the following credit card information on fax requests:

• Name of credit card holder
• Type of credit card (Visa, Mastercard, American Express, Discover)
• Account number
• Expiration Date
• Amount to be charged
• Signature of credit card holder
• Billing zip code of credit card holder

Please be aware that if you are paying for your transcript with a credit card, transcripts will not be prepared until the charge has been authorized by the credit card issuer.
* - Processing time does not include mailing time.

UNOFFICIAL TRANSCRIPTS

Available on the web at moorparkcollege.net/webstar or at the Moorpark College Records Office. Cost: Web transcript - no charge. In person: $1.00. Picture I.D. required.

FEES/OBLIGATIONS

Transcripts cannot be released if student has any unpaid fees or obligations.
