Counseling Checklist – Bring with you to your appointment with the counselor, along with all transcripts and supporting documents.

Moorpark College Nursing Application Checklist (Counseling Checklist)

Download the application from the “Forms” section of the nursing webpage and fill it out completely. Submit all materials stated below in full with NO EXCEPTIONS. Any incomplete applications will not be considered.

 
Prerequisites for the Nursing Program

* Complete all courses stated below with a grade of “C” or higher and have a cumulative Grade Point Average (GPA) of all college coursework of at least 2.5. In addition, all core sciences and intermediate algebra must have been completed within the past five years.

English 1A (4 units)

Math 3 Intermediate Algebra (5 units) or higher

Chemistry 11 (5 units) or high school Chemistry, please refer to counselor (if not applying to CSUCI, Chemistry 12 is accepted). Prerequisite to  Physiology 1 and Microbiology 1.

Biology 1 or 2A (4-5 units) (Biology 2A preferred)

Anatomy 1 (4 units)

Note: Anatomy/Physiology combined course is not accepted, unless a two part, two semester course

Physiology 1 (5 units)

Note: Anatomy/Physiology combined course is not accepted, unless a two part, two semester course

Microbiology 1 (5 units)

Copy of CPR through the American Heart Association: “BLS for the Healthcare Provider”. (Not required to apply but need to have before starting the Nursing Program)

 
LVN-RN or Transfer Student Application Process

• Acceptance based on space availability
• Same prerequisites as above

SUBMIT with Application:

OFFICIAL HIGH SCHOOL TRANSCRIPTS (if you don’t have any college degrees: AA, AS, BA, BS, MA, MS, Evaluated International Transcripts). Copy of Diploma not accepted.

ALL OFFICIAL COLLEGE TRANSCRIPTS (in-state, including Moorpark, Oxnard, and Ventura Colleges, out-of-state, and/or Evaluated International Transcripts). Please attach course descriptions from the college where you took the course during that semester and year.

 
Personal letter from applicant requesting admission: Identify the course requested for admission, explain applicant’s current situation and future goals, and outline any activities, remediation or work experiences completed to prepare for admission eligibility.

1. Licensed Vocational Nurses also submit:
   • Copy of current LVN license
   • Resume
   • Two letters of recommendation from current LVN employer and Nursing Director/faculty from previous program
   • PN Comprehensive Predictor exam score (please contact Carol Fox (805) 378-1400 Ext. 1876 to schedule an exam date)
2. Transfer students from other nursing programs also submit:
   • Previous nursing school course syllabi and college catalog
   • Two letters of recommendation from previous nursing school faculty and/or nursing director
   • TEAS V test score of greater or equal to California Community College Chancellor’s recommended minimum (≥62%)
3. Readmission applicants to Moorpark College also submit:
   • Proof of volunteer or work experience
   • Remediation academic work
   • TEAS V test score. If score is <62%, all remediation must be completed prior to application. Proof of remediation must be submitted with application.
