Instructions
- Authorization To Release Information (PDF)
- This form is to be used when you need Moorpark College to formally certify your enrollment and/or GPA for any reason including health insurance, good student automobile insurance discounts, and loan deferrals.
Because your academic records are private and confidential, they cannot be released without your written consent as provided on this authorization. Complete the authorization making sure you identify what action you need us to take and attach any forms you need us to sign. Sign, date, and return the authorization with the appropriate processing fee to the Office of Admissions & Records. Please be aware of the processing time required for completion of your request as stated on the authorization.
- Pass/No Pass (formerly Credit/No-Credit) Request Form
- This form is to be used for any class in which you prefer not to receive a letter grade (A, B, C, D, F). It is generally used for classes outside of your major or classes that will not transfer to a 4-year university. However, students are advised to check with their counselor before submitting this request.
If the grade you would have received in the class is A, B, or C, you will receive the grading symbol of "CR". The units for the class will count towards your unit total but will not affect your grade point average.
If the grade you would have received in the class is D or F, you will receive the grading symbol of "NC". The units for the class will not count toward your unit total and will not affect your grade point average.
Please read this request carefully and provide all requested information. Submit the request to the Office of Registrations & Records by the deadline date posted in the Schedule of Classes or on the college website.
- Course Repetition Form
- This form is used to notify us that you are repeating a class in which you previously earned a D, F, or No-Credit grade. Once processed, the original grade will be excluded from the computation of your grade point average and your transcript will be annotated indicating repetition.
Please read this form carefully and provide all requested information. To make sure you list the correct semesters on the form, please access your unofficial transcript at moorparkcollege.edu/webstar.
Once you have completed the form, submit it to the Counseling Department for review. Upon approval, the form will be forwarded to the Office of Admissions & Records for processing. Please be aware that the grade for your repeated class must posted to your transcript before the petition can be processed.
- Petition to Change Grade to Withdrawal
- If you have an "F" on your transcript and feel you have extenuating circumstances that would support changing the "F" to a "W" (withdraw), you may use this petition. This petition requires that you list and describe your extenuating circumstances for requesting the change.
Please read the petition carefully and provide all requested information. Attach any documentation supporting your request.
Return the petition to the instructor of the class you are petitioning. This petition requires the approval of your instructor, his/her Division Dean, and the Dean of Student Learning. Once approved, it will be forwarded to the Office of Admissions & Records for processing. The entire petition process takes approximately 3 weeks, longer in Summer or if the faculty member is no longer employed at Moorpark College.
If you would like a receipt indicating if your petition was approved or denied, please submit two copies of the petition instead of one. The 2nd copy will be returned to you after it has been processed to the address we have on file for you.
- Petition to Drop Class After the Final Drop Deadline
- If you did not drop a class by the final drop deadline, you may use this petition to attempt to drop it after the deadline. This petition requires that you list and describe your reason(s) for not being able to drop a class by the deadline.
Please read the petition carefully and provide all requested information. Attach any documentation supporting your request.
Return the petition to the instructor of the class you are petitioning. This petition requires the approval of your instructor, his/her Division Dean, and the Dean of Student Learning. Once approved, it will be forwarded to the Office of Admissions & Records for processing. The entire petition process takes approximately 3 weeks and may take longer in the summer.

- Special Admission Form
- This form is to be used for students under the age of 18 who are still enrolled in high school, middle school, or grade school. Registration restrictions apply and they are listed on the form.
Please read this form carefully and follow all instructions. Once you have the required signatures, refer to the Moorpark College Schedule of Classes for the registration date that applies to you. Special Admission students must register in person although a parent may complete your registration for you. Special Admission students must also comply with all mandatory pre-requisite requirements if they are attempting to enroll in English and/or Math classes. A transcript from your current school is required.
If you are new to Moorpark College, you will also need to submit a completed application for admission along with the signed and approved Special Admission form.
An updated and approved Special Admission form will be required for every semester you attend until you graduate from high school.
