How to apply:

ACCESS is accepting applications through email. Please complete steps 1 and 2 and let us know when you’d be available for a meeting. We will contact you to  schedule an intake appointment either online, over the phone or on campus.

Steps to follow:

Step 1 □ 

Complete the Application for Services and Intake Screening form

Step 2 □ 

Submit copies of your Verification of Disability

(Email completed copies to MCACCESS@VCCCD.EDU)

  • Students with an IEP or 504 plan, submit a complete  (all pages) and most current copy of your IEP or a copy of a recent psycho-educational report upon which services were determined (Medical/Other Health reports, etc.)

  • If you have ADHD, a medical, physical or psychological disability, you will need a Verification of Disability Form completed by your doctor or psychologist (please print the form in the following link and have your doctor complete, sign and stamp the form)

Step 3 □ 

Once steps 1 and 2 have been completed, we will call you to set up an intake appointment

   

Step 4 □ 

If you are a current high school student or recent graduate, submit an unofficial transcript as well.

ACCESS is located on the first floor of the LM building. This is a two story building at the northeast side of the campus. You may park in Lot C or the handicapped parking lot next to the building. Campus Map
